Chile[]
ThyssenKrupp elevator incident[]
- Location: Tower B, Bustamante Community Park, Providencia, Chile
- Date of incident: June 8, 2014
On June 8, 2014, a ThyssenKrupp elevator in Tower B of Bustamante Community Park in Providencia, Chile, suddenly malfunctioned, then rises 30 floors in 15 seconds with the doors still open before crashed into the shaft's roof. A man was seriously injured, and was reportedly that he suffered head and leg injuries[1] [2].
It was suspected that the brake was picked (opened up) and there was a loss of motor control which cause the elevator into free rolling into the opposite direction.
In addition, the elevator didn't have an unintended car movement protection device (Rope Brake) or an ascending car overspeed protection device (overspeed governor), which means that the elevator "fell" upward since the counterweight weighs more than the elevator cab.

Shenzhen elevator accident[]

On May 15, 2013, a nurse was decapitated after being caught between the doors of an one of the four Mitsubishi elevators and was dragged along for at least 3 floors. It was reported that the elevator was going down 22 floors when it stopped suddenly between levels 2 and 3. She then tried to exit the elevator, only to be trapped between the closing doors[3][4]. The Wellift elevator company has been held 100% responsible for the incident and was ordered to make compulsory maintenance to all elevators maintained by the company.

Kone Marryat Scott PRH elevator downfall[]

On October 25, 2008, a Kone Marryat & Scott elevator in Shin Nga House, Fu Shin Estate, Hong Kong suffered from a downfall incident, with the faulty sheave on the counterweight. ThyssenKrupp was held 100% responsible for this incident[5]. Finally the elevators in this housing estate were replaced by Otis.
HKDI campus escalator incident[]

On February 24, 2011, some of the escalator landing steps on one of the two ThyssenKrupp escalators at Hong Kong Design Institute (HKDI) in Tseung Kwan O that gives access from level 1 to 7 fell down on the steps which were returning to the seventh floor. The escalator safety device was activated when the steps were falling down[6]. No one was injured in this incident.
North Point restaurant elevator downfall accident[]

On March 2, 2013, a Mitsubishi elevator in a restaurant plummeted down from 1st floor[7] due to governor fault and wire rope issues[8]. All 7 passengers got injured[9]. Shineford Engineering was held 100% responsible for this incident and EMSD force their "Registered Lift and Escalator Contractors" License cancelled[10]. Finally this elevator being replaced by Schindler (5400 AP MRL) in May, 2014[11] and opened in September, 2014.
Elevator over-travel incident in Tsuen Wan (2014)[]

On January 25, 2014, A Dong Yang elevator in Waterside Plaza Block 1 breakdown and over-travel to the machine room during maintenance. It caused the machine room damaged[12]. The maintenance contractor, Ben Fung was fined HK$3000 by the Court.
Kwun Tong Industrial Building elevator downfall accident[]

- Incident nature: Passengers overloading the elevator
On October 8, 2014, A Falconi passenger elevator (which was held a major alteration in 2011 by third-party elevator maintenance company, Nikkin) in Selwyn Factory Building plummeted down from 12th floor by passengers overloading the elevator, 29 of 31 passengers got injured when 2 passengers already escaped when it begin to downfall (the maximum loading of this elevator are 1600 kgs, or about 21 persons)[13]. EMSD needs to increase public awareness of the safe use of lifts and the proper way to prevent overloading of the elevators after this accident[14] such as the new advertising half of the years before[15]. The contractor, thyssenkrupp was fined HK$180,000 (total four discipline cases, fined HK$45,000 in each) and to pay the costs and expenses incurred in the proceedings totalling HK$136,342 for its improper maintenance[16]. Finally this elevator being replaced by thyssenkrupp in November, 2015 and opened in early 2016[17]. The elevator car size has been reduced to comply the latest (EMSD Circular No. 12/98) Available Car Area requirement[18].
Langham Place escalator out of control accident[]

A long, 45-meters Otis escalator at Langham Place which ran from the 4th floor up to the 8th floor malfunctioned; it suddenly stopped, reversed and then sped up, causing riders to lose balance, fall and piled up on the bottom. Eighteen people were injured, with one in serious condition. Investigations carried out by the Electrical and Mechanical Services Department (EMSD) revealed that the malfunction was suspected to have been caused by a faulty drive chain, and the safety brake system failed to work[19][20]. The escalators are maintained by Otis Elevator Company (HK) Ltd. The two company technicians were arrested because of preventing the course of justice as they tried to control and repair it and move the components of the escalator before the accident investigation done by EMSD. Finally, Otis was fined HK$320,000 (total four criminal cases, fined HK$80,000 in each)[21][22].
Elevator over-travel accident in Tsuen Wan (2018)[]

On April 8, 2018, A remaining 1991 Dong Yang elevator in Waterside Plaza Block 2 breakdown and over-travel to the top floor. The couple got seriously injured while the ceiling fell down when the elevator car hits on the top of the shaft[23][24]. The elevators in this housing estate are maintained by KONE Elevator (HK) Ltd., while the other elevators being replaced by this company[25]. Due to the malfunctioning of the brake system, the ascending car overspeed has occurred[26]. However, the criminal investigation of this accident is still pending[27].
Unintended car movement accident in Sheung Shui[]

On May 11, 2018, a Guangri elevator in Paris Court, Sheung Shui Town Centre broke down and over-travelled to the top floor. An old woman was trapped between the shaft door and car door; she died when she fell into the bottom of the shaft[28][29]. The ascending car overspeed was due to a malfunction of the brake system.[30][31]. The maintenance contractor, Eugene Engineering Co. Limited (subsidiary of Hitachi Elevator in Hong Kong, China) was fined HK$63,000 and its General Manager was fined HK$33,000[32].
Indonesia[]
Ratu Plaza elevator downfall[]

On November 16, 2007, a Hitachi elevator which served the two underground floors of Ratu Plaza Office Tower in Jakarta fell down from six meters high and injuring four people. It was reported that the accident was caused by broken suspension ropes. The building manager was held responsible for the accident[33]. The elevator has since been replaced into a new Mitsubishi MRL elevator[34].
2014 Monumen Nasional elevator breakdown[]

On May 4, 2014, a series of breakdowns happened on a Hitachi elevator at Monumen Nasional (Monas). and it got malfunctioned and trapping passengers inside. It was later fully repaired and maintained by PT. Pillar Multi Sarana Utama (a distributor of Guangri elevators in the country), and was finally replaced into a modern Hitachi elevator in late 2013[35].
2014 Kemayoran elevator freefall accident[]

On Sunday, March 9, 2014, one of the two Hyundai elevators at Rusun Kemayoran Tahap 3, Kemayoran, Jakarta fell down freely from the 4th floor after the elevator was overloaded with 13 passengers, which supposedly it only carried up to 9 persons. It was reported that the overload detector and buzzer weren't working. Passenger were also warned by the building's operator to not exceed the elevator load but they ignored it. All of the passengers were injured, mostly with fractured bones[36] [37]
2015 Taman Kemayoran elevator downfall incident[]

- Date of incident: September 30, 2015
At around 1 o'clock afternoon on September 30, 2015, one of the 1990s Hyundai elevators in Cendana Tower of Taman Kemayoran Apartment in Kemayoran, Central Jakarta[38], fell from the 6th floor after it picked up passengers from the 8th floor. Nine Indian people were injured and then treated to Mitra Kemayoran Hospital[39].
The maintenance contract for the elevators was held by PT. Raka Ageng Vetra as of September 2, 2015. According to the building's announcement on October 1, 2015, it was reported that when the car overran to the buffer, the safety brake didn't work properly as the 12 mm sling/wire rope on the overspeed governor had decreased due to its age[40]. The safety circuit, however, was working properly.
2015 Arkadia elevator incident[]
- Location: Nestle Building, Building B of Perkantoran Hijau Arkadia, Pasar Minggu, South Jakarta, Indonesia
- Date of incident: December 10, 2015
At 9:40 AM on December 10, 2015, an Otis Spec90 elevators designed for PT. Nestle Indonesia employees fell from the 7th floor to the 3rd floor, killing two Nestle employees and injuring one ISS employee[41]. The incident also forced people to evacuate from the building because of the loud bang and vibration caused by the free-falling elevator. Some said that they felt it like an earthquake[42] The elevator has since been replaced into a new Otis ReGen elevator.
Apartemen Taman Rasuna incident[]

An elevator at Tower 1 of Apartemen Taman Rasuna has fallen to the B3 floor. There were 5 people from the 5th floor then it caused a screeching sound (the brakes). This was right after the TK mod and the elevator affected was the big service elevator.
Hospital elevator downfall[]

An elevator in the Lotus Building (Gedung Teratai) of Fatmawati Hospital (RSUP Fatmawati) suddenly fell from the 4th floor to the 1st floor due to a loose brake. All the passengers inside were injured. The maintenance company responsible for maintaining and servicing all the elevators in the hospital was PT. Megah Daspa Sentosa, which was said to be unlicensed[43].
Fujitec elevator downfall[]

- Date of incident: Friday, January 20, 2017
A Fujitec elevator in the BRI II Building suddenly fell from the 4th floor to the 1st floor in the afternoon of January, 20th 2017 before friday prayer and during lunch break due to rope broke and overshooting. All the 11 passengers inside were injured. The elevator rope broke when it was on the 11th floor. However, luckily it didn't hit the bottom right away. Because, had stopped on the 3rd floor, and fell back.[44]. Witnesses said the elevator stopped on the 3rd floor during freefall before hitting the bottom floor[45]. It has since been replaced with newer Fujitec elevators.
Blok M Square elevator downfall[]

- Date of incident: Friday, March 17, 2017
Elevator number 5 of Blok M Square, Jakarta, Indonesia fell down from the 7th floor in the afternoon of March 17, 2017. The accident was caused by overloading; the elevator, which could only carrying up to 24 persons or 1600 kilograms, was filled with 25 people, although some sources said that 35 people were entering the elevator. Witnesses said the elevator stopped on the 3rd and the 1st floor (because of safety brake got triggered) during freefall before hitting the bottom floor, injuring 25 people inside[46][47]. It has since been replaced with newer generation Sigma elevators.
Grand Indonesia elevator downfall[]

Elevator number BP3-01 of Grand Indonesia, Jakarta, Indonesia fell down from the 3rd floor in the evening of December 25th, 2017. The accident was caused by twisted wires. this elevator was stuck for an hour due to a cable problems; the elevator, which could only carrying up to 21 persons or 1600 kilograms, was filled with 9 people, with 4 of them are Indian. Public Relations of Grand Indonesia admitted that there were obstacles in the elevator. However, the safety break lift can work normally so that it can be held up and does not plunge to the ground floor. One of the eyewitnesses, said there were children and women who were trapped in the elevator on the UG floor, trapping 9 people inside which was later survived. The victims looked traumatized after being rescued. Some of them even had difficulty breathing and had to wear oxygen masks.[48][49]. It has since been replaced by a Fujitec ZEXIA.
Pasar Pagi Asemka downfall[]

A generic Pillar elevator at Pasar Pagi Asemka fell down. It turned out that the elevator was overloaded since there is a footage of the incident that the elevator indicator showing "OL" is seen. It has since been replaced by a Lemova, then a Schindler 5500 because of the incident.
Tanah Abang elevator breakdown[]

On January 16, 2020, an elevator breakdown happened on a Hyundai elevator at Metro Tanah Abang. and it got malfunctioned and trapping passengers inside. Elevator technician and Policeman said that elevator was stuck due to machine breakdown and there is a bent iron. there were no casualties or injuries in this incident. Everyone who was in the elevator was successfully rescued manually. However, some people, mostly women, went into shock as a result of the incident.[50]
2020 DPR/MPR elevator fire incident[]

On September 29, 2020 afternoon, an elevator fire incident happened on a Otis Elevonic 411 elevator at Wisma Nusantara 1 Building inside DPR/MPR Complex. and it suddenly smoky during refurbishment. Thick smoke was also seen blanketing the basement floor of Nusantara I DPR Building, fire alarms also had sounded. The elevator for members of the House of Representatives on the basement floor is known to be in the process of repair since a few months ago.[51]
Margo City elevator incident[]

A ThyssenKrupp Dongyang freight elevator in Margo City shopping mall in Depok, West Java, fell from the third floor after an explosion caused by a gas leak. The explosion occurred at 4:30 p.m. on Saturday and caused the mall's ceiling and walls to collapse. The blast triggered panic as workers and visitors assumed it had come from a bomb. Local Police chief Comr. confirmed that there was no bomb and an elevator had fallen. Police bomb squad has checked the area and there was no bomb. The Local Police forensics laboratory concluded that a gas leak had triggered a blast, which subsequently caused a freight elevator to fall from the third floor due to its heavy load and the building's fragile structure. One person, a coffee shop employee, later died in the hospital early on Sunday after sustaining head injuries from falling debris. Ten others were injured, including mall employees, security guards and visitors. Police said they will continue to investigate the cause of the gas leak, and the mall will be closed until further notice.[52] Later, the mall reopened in September, 4th 2021.[53] It's unknown what happened to the elevator after the incident, but rumors stated that it will be most likely replaced by a Mitsubishi NEXIEZ.[53]

2023 Ayuterra Resort inclined elevator downfall incident[]
An old typical vintage-styled inclined elevator at Ayuterra Resort in Ubud, Bali, slides down like a rollercoaster after it almost reaches to its desired floor, causing all 5 repairmen to fall down dead. It turned out that the resort owner intentionally reduces 3 wired ropes to only 1. It's unknown what happened to the inclined elevator after the incident.

2004 Nagoya elevator incident[]

A Schindler elevator in Nagoya descended while the door was still open. Nobody was injured in this incident. Investigation found that elevator's firmware "TV60 v1.1" had a bug that opens the door when elevator starts moving.
2006 Tokyo elevator incident[]

Three passengers were trapped in the elevator in Tokyo, after the elevator's door opened while it was ascending. Elevator had faulty firmware "TV60 v1.0". The elevator is identified to be a Schindler.
2006 Minato elevator incident[]
- Location: Minato Ward, Tokyo, Japan
- Date of incident: June 2006
A Schindler elevator in Minato, Tokyo, started ascending while the door was open. A 16-year-old boy was crushed to death while getting off the elevator with his bicycle. An investigation determined a faulty brake was to blame. When the government investigation team requested Schindler to disclose the information about elevators installed, Schindler declined to comply with the request and investigation claiming that documents contained personal information.
2006 Chiba elevator incident[]

A Schindler elevator in Urayasu, Chiba started ascending while the door open, passed the top floor, and finally stopped in the mechanical shaft. Two people were trapped in the car. The elevator had firmware "80TH v1.4" with faulty codes.
2010 Tokyo University incident[]

- Date of incident: 2010
18 students at the Kashiwa campus of University of Tokyo fell from ground floor to the basement floor in a Schindler elevator with the doors still open. One student incurred a minor injury while trying to escape. Schindler determined the cause to be loose brake wires.
Hotel Kanazawa (Kanazawaekimae) elevator incident[]

A 63-year-old woman at APA hotel in (known as APA Hotel Kanazawa Eikmae or APA (ホテル金沢駅前)in Kanazawa (Ishikawa Prefecture), was crushed to death between the ceiling and the floor, when the Schindler elevator started moving while the door was still open.

A Mistubishi MRL elevator at Tokyo Metro Heiwadai station falling down when elevator go upwards and then elevator stopped at 3 meter from buffer and 50-year-old woman injured. the investigation found the all 3 ropes has get cutted off. the rust has been found since September 2010, but the rope are not replaced due not reached the criteria that needs to be replaced.

PPR Kerinchi elevator incident[]

A Dover (Malaysia) elevator in PPR Kerinchi Block C fell down from the fifth floor to the ground floor. With the drop estimated to be at a height of 15 metres, it is believed the elevator car's braking sytem had malfunctioned in the incident at 2.30pm on that day. Nine people including one woman, a child and a disabled man were in the elevator that time. Eight of nine people were injured with four suffered broken legs. The cause of the accident was believed to be a brake failure caused by faulty mechanical components.Both brakes functioned to stop the car and secured it to a particular floor. During the incident, one of the two brakes had failed to activate, after stopping on the fifth floor. The car then began hurtling down at 2.13 metres per second (m/s), which was beyond its normal operating speed of 1.75m/s.The lift was equipped with a backup brake that did not trigger during the incident.The elevator's back-up brake would only trigger if the car exceeded the 'tripping speed', which is 2.18m/s.The elevator's mechanical components were operated by a solenoid pair, a type of electromagnetic device used to convert electrical energy into mechanical energy. The elevator's solenoids were taken out for inspection by the maintenance contractor a week before the accident,however, no faults with the solenoids were identified during this time. Dover Elevator (M) Sdn Bhd was later fined RM28,000 for the accident.

Never maintained older Sovietic elevator modernized in 2001 by Otis MCS-300[]

On December 19, 2015, an older Soviet elevator modernized in 2001 by Otis MCS-300 at Ostrovityanova 49 in Moscow, Russia, young mother rolled her stroller into the elevator and the elevator suddenly broke the rope and as a result of the arrival of one floor stroller ripped and 10 month old girl died.

Damaged Fujitec elevator in Tampines[]
- Location: Block 17 Marsiling Lane HDB, Singapore
- Date of incident: June 9, 2013
On June 9, 2013, a Fujitec elevator at Block 17 Marsiling Lane HDB, Singapore was badly damaged with the landing doors on the ground floor dislodged from the door tracks. It was reported that the elevator damage was due to a door alignment defect, which was suspected to be due to vandalism, but it turns out that the damage was suspected to be from misuse by workers. A spokesperson for Fujitec said that workers who move materials in and out of the elevator would use a piece of cardboard or wood to wedge the doors open. This repeated force into the gap, would cause the safety edge to become loose, and lean outwards towards the landing doors and get stuck when the elevator goes upwards, causing the doors to be dislodged from the door tracks.
Malfunctioned Otis elevator in Tampines[]

On May 2013, an elevator in a HDB block in Tampines suddenly ascended with the doors still open when a five years old boy was about to exit the elevator. He was pulled back by his father. After ringing the alarm bell repeatedly , the elevator doors closed and the elevator ascended to the top floor and became out of order . The elevator was identified to be Otis.
Bukit Batok West elevator breakdown[]
- Location: Block 115 Bukit Batok West HDB, Singapore
- Date of incident: October 18, 2010
On October 18, 2010, an elevator at Block 115 Bukit Batok West HDB which was identified to be a ULift broke down and caused two children to be trapped inside for an hour. Both youngsters were not injured. An investigation had shown that the interlocks have been misused by renovation workers renovating a flat. The renovation company was fined S$2000 and was ordered to pay for the repair works on the elevator.
Series of breakdowns on upgraded EM Services elevator[]
A series of breakdowns on a batch of upgraded EM Services elevator has been caused by a misalignment of the door's interlocks, which would occasionally get stuck. EM Services has fixed the elevators and Shenyang Brilliant elevator was held responsible and had to bear the full cost of the repair works, with an additional $100 fine for every elevator fixed.
Faber Crest Condominium elevator incident[]

A Hitachi elevator in Faber Crest Condominium had overshot and hit the roof in December 2008, thus damaging the machinery badly. In 2009, the elevator was re-installed into a small machine room (SMR) elevator.
Marina Bay Sands Tower 2 elevator incident[]
A Kone elevator at Marina Bay Sands opened its doors midway on its journey to the first floor. The doors opened slightly before coming to an abrupt stop and continued jerking downwards for 3 floors. The elevator then free fell 2 floors before returning to normal service. Taman Jurong elevator incident[]

- Date of incident: October 9, 2015
On 9th of October 2015 at 7.27am , a woman's hand was severed by an elevator at Block 322 Tah Ching Road HDB, located in Taman Jurong, western Singapore[54]. The woman's dog was outside the elevator and the elevator doors closed and ascended. As the dog leash was strapped over her hand, the force of the ascend severed her wrist. The elevator was a GoldStar elevator that is maintained by Sigma Elevator Company Singapore. The incident has prompted a Committee Of Inquiry and a full report is out. The woman involved is in stable condition. The sensors on the lift was found to be working and safety devices were in good condition. The elevators at the block have since been replaced in 2022-23, due to age and not at all related to the incident.

Östermlamstorg Metro Station escalator incident[]

An elderly woman fell victim to a serious escalator incident on February 16, 2015 at Östermlamstorg Metro Station. The escalator was built by Asea-Graham in 1965 but had obsolete and light-weight aluminium steps custom made by a company called JURK service AB. One of theese steps had its axis broken apart and the step fell through, causing the woman to get stuck inside. She received serious injuries. This accident hit the news hard, consequently causing all the ASEA-Graham escalators with the JURK steps to get turned off for a substantial period of time for emergency maintenance and prompted an increase in security and protection, avoiding more injury if more steps broke down.[55]

On March 12, 2003, a banker was crushed to death by a ThyssenKrupp elevator which was not properly maintained in a health club in London, England. The health club owner and ThyssenKrupp were fined £466,000 after the accident.

On November 21, 2018, an Otis express elevator carrying six passengers fell about 84 floors after picking up passengers from the restaurant on the 95th floor. The elevator stopped between the tenth and eleventh floors. Passengers reportedly heard noises, which were two of the four cables of the elevator breaking.[56]

On Sunday of January 20, 2019. An employee got stuck and got injured by an stalled elevator at an Hospital in Ft. Worth. The elevator was built by Montgomery in 1993 and was maintained by Thyssenkrupp Elevator Company. After the incident was occurred. The elevator with a decal 27129 was removed from service.[57]
